COVID-19 Waiver

1. I hereby release, waive, and discharge Dream Big Project, team members, ambassadors, board members, influencers, and all participants, including sanctioning organizations or any affiliate, subsidiary or subdivision thereof, officials, car owners, drivers, rescue personnel, sponsors, advertisers, owners and lessees of premises used to conduct the events, premises and event inspectors, surveyors, underwriters, consultants and others who give recommendations, directions, or instructions or engage in risk evaluation or loss control activities relating events and for each of them, their directors, officers, agents, and employees, all for the purposes herein referred to as “RELEASEES," from all liability to me, my personal representatives, assigns, heirs FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIM OR DEMANDS THEREFORE ON ACCOUNT OF INJURY TO THE PERSON OR PROPERTY OR RESULTING IN THE DEATH OF THE UNDERSIGNED ARISING OUT OF OR RELATED TO THE EVENTS, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. In addition, I COVENANT NOT TO SUE any of the RELEASEES based upon any claim arising out of any of the events. 2. I hereby ASSUME FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE arising out of or related to the EVENTS whether caused by the NEGLIGENCE OF RELEASEES or otherwise. 3. I hereby AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost they may incur due to claims brought against the releasees arising out of my injury, or death, or damage to my property while I am participating in the activities and events and/or while participating, practicing, and officiating.

Liability and Affirmation Waiver

I hereby affirm that my answers to questions on the application are true and correct, and that I have not knowingly withheld any fact or circumstances that would, if disclosed, affect my application unfavorably. I understand that any false information submitted in this application is cause for denial of this application or termination of my volunteer services regardless of when or how discovered. I hereby waive any right to claim that any request or investigation is an invasion of my privacy, since they are made with my consent and it is in my best interest while being considered for a volunteer position. I hereby acknowledge that I agree to donate my services to the organization for the period of time that I am volunteering; I hereby acknowledge that I am not expecting and waive my right of any present or future payment for the work that I do during my volunteer service with the organization; I hereby acknowledge that I am not an employee of the organization; I hereby acknowledge that I am personally liable for any damages that I cause to third parties for anything that I do outside of my volunteer work; I hereby acknowledge that I am personally liable for any damages that I cause to third parties for anything that I do outside of my volunteer work if I were negligent; I hereby agree to confidentiality and nondisclosure terms about sensitive and confidential nonprofit information; I hereby agree that any work performed while volunteering for the organization becomes the property of the organization; I hereby agree to complete any required training and follow the rules of the nonprofit. I have read and understand the above statements and that I voluntarily sign this affirmation.

Photography and Video Waiver

I hereby affirm that by volunteering with Dream Big Project I will be entering spaces where my photo and video recording will be documented. I hereby affirm that photos and videos of me volunteering with Dream Big Project will be printed on marketing materials and presented including but not limited to social media posts, company website, brochures, flyers, signage, and media presentations.
